I hereby acknowledge that I have voluntarily and freely elected to participate in Pinnacle Hoops training or camps and that I am not required to do so. I understand and agree that Pinnacle Hoops and/or its representatives assume no liability in the event of accident or illness, nor for damage or injury to person or property of any nature whatsoever. I voluntarily and freely assume all risks of accident, injury, illness, or damage to or loss of property. Pinnacle Hoops shall not be responsible to any person for any of my acts or omissions. I agree to release, indemnify, and hold harmless Pinnacle Hoops from and against any claim in which parents, my guardian, of any other person or I, may have for any losses, damages, or injuries arising out of or in connection with my participation in any Pinnacle Hoops trainings or basketball camps.

I do understand the risks of the possible spread of COVID-19 through the Pinnacle Hoops training and do summer camps and do not hold Pinnacle Hoops or their trainers at fault for any COVID-19 caused problems. I agree that all trainers will be wearing masks at all times and if my child or I have had any COVID-19 symptoms or been in contact with anyone having COVID-19 within the past 2 weeks please let our team know ASAP and do not come to any further trainings or camps. We thank you in advance for your help in stopping the spread of COVID-19 and keeping all our loved ones safe.
